Physical Examination for Freshmen in 2021

• All the Freshman are required to complete the physical examination before the end of this semester in 2021. Even you are an ARC (Alien Residence Certificate) holder, you still need to do the “Freshman Health Examination.”
• If you want to do the physical examination in NCKU hospital, NCKU has provide two types of physical examination for 2021 Overseas Freshman to choose (General-NT$850/ For visa-NT$1300), please choose one of them after thinking twice or discussing it with OIA (Office of International Affairs) staff.
• Be noticed, you’ll need to provide your physical examination report to the NCKU Health Center if you are not doing the examination during the period which your college/department has discount provided by NCKU hospital.

1.  Types of physical examination

(1) General Physical Examination -NT$850

• Valid period: Oct. 18th, 2021- Jan. 31th, 2022(excluding weekend and holidays)

• Please follow the steps below to get your voucher and remember to complete the physical examination in this semester.

(2) Physical Examination for visa-NT$1300

• Be aware, if you only have “visitor visa” and need to apply the resident visa for ARC, you may need to choose “Physical Examination for visa”

• Valid period: Oct. 6th, 2021- Jan. 31th, 2022( the date is assigned by NCKU Health Center)

• Because there is daily limit number of “physical examination for visa (NT$1300)” in NCKU hospital, we'll schedule your date of examination according to the expiry date of your visitor visa and the dates you choose on the google form below.

• Please follow the steps below to get your voucher and remember to complete the physical examination on the date we assigned for you.

2.  Booking way of physical examination
 

Because each student arrived Taiwan at different time, please check the following physical examination registration process according to your condition.

(1)  If you came to Taiwan after May. 1^th, 2021

Step 1
Please complete the “Freshman survey” and print out the consent (NCKU for Freshman Physical Examination Consent for Release of Information) on single side(include two copies) in “Student Health Curriculum System”.

Step 2
Make sure the last date of finishing Home-Quarantine(14-day) and Self-Health Management(7-day) when arriving Taiwan. And also make sure which type of physical examination you want to take by asking the OIA staff for help. (Tel: 06-2757575#50990)

Step 3
(1) If you need the physical examination for visa(NT$1300), please check “ Schedule of Physical Examination for visa” list on NCKU Health Center website before you fill in the google form(set by NCKU Health Center). We'll start to arrange the date and update the Schedule.  
(2) As for students who want to do the “General Physical Examination”, please fill directly in the google form for making an appointment. We'll start to arrange the date of examination for students.

Step 4
(1)If you came to Taiwan between 5/1-10/3, remember to notice your department or dormitory for requesting a kit of Covid-19 self-test, do the test by yourself on the last day of Self-Health Management(21-days after arriving Taiwan) and upload the pictures of the self-test's result to the『KUAP app』
(2) If you came to Taiwan after 10/4 (including 10/4), remember to keep the Covid-19 self-test kit which gave by CDC person on the arrival date, and use it on the last day of Self-Health Management (21-days after arriving Taiwan). Please don’t forget to upload the pictures of the self-test's result to the『KUAP app』and report to the 『CDC message』.

Step 5
After the self-test result you uploaded has been checked by the staff in NCKU Health Center, you can print your voucher on “Online Pre-arrival Survey for NCKU Overseas Students”by yourself.
And be aware, if you choose to do “physical examination for visa”, please update the date which NCKU Health Center have arranged for you before you print your voucher of physical examination

Step 6
Bring the voucher and other files you need when going to the hospital on the date of physical examination.
Register time starts from 08:13-11:00 in the morning and 13:03-16:00 in the afternoon. And be aware, there's no offer for weekends and holidays.

(2)  If you came to Taiwan before May. 1^th, 2021

Step 1
Check the website of NCKU Health Center for knowing more instructions of making an on-line appointment of physical examination with NCKU Hospital Reservation System.

Step 2
Please complete the “Freshman survey” and print out the consent (NCKU for Freshman Physical Examination Consent for Release of Information) on single side(include two copies) in “Student Health Curriculum System”.

Step 3
(1) If you choose to do “General Physical Examination”, please making an on-line appointment in “NCKU hospital reservation system” according to discount period of your college and department.
(2) If you choose to do “Physical Examination for visa”, please fill in the google form and we'll schedule your date of examination.

• The freshman physical examination discount period at NCKU hospital for new master, and doctoral students in the fall semester of 2021 academic year (excluding Saturday and Sunday)

• The freshman physical examination discount period at NCKU hospital for new undergraduate students in the fall semester of 2021 academic year (excluding Saturday and Sunday)

3.  What you need to prepare for Health Examination

• Required documents： 

           (1) Admission letter

           (2) Passport / Visitor Visa

           (3) Fee- NT$850/NT$1300

           (4) Voucher of physical examination (only who came to Taiwan after May, 2021 or other special situation needs)

           (5) First Visit Registration Form 

           (6) NCKU for Freshman Physical Examination Consent for Release of Information (Please print it single sided)

• Registration Time：
  Monday to Friday 08:13~11:00(a.m.), 13:03~16:00 (p.m.)

• Health examination remarks:

1. Please fast for 12 hours before the health examination. If you are really thirsty, you can have some water, but not any drinks with sugar.
2. Please wear loose and comfortable clothing to attend health examination.123
3. For students on prescribed lenses, please bring the lenses on the day of the examination, so that post-treatment eyesight can be tested.

• Sending of Report：
  Once you finish the whole procedure, the hospital will send the report to you in 1 month；if you choose “Physical Examination for visa”, the hospital will send the report to you in 10 days.
